BAND

As soon as possible! All students must be updated prior to the beginning of Band Camp in July.  If you do not update your student at least 2-3 weeks before Band Camp, we can't guarantee they will be cleared in time and may be sitting out until we can get them cleared.

Please log in to your FinalForms account to view this information.  Upon logging in you will see documents highlighted in green, which indicates they are complete. Items highlighted in red, indicate they are incomplete. Once all items are highlighted green, the student-athlete is cleared.

Coaches will only be able to see the athletes who have signed up for their particular sport. You must add your student to Band.

No! Liberty Public Schools will not collect any medical documentation. All required documents must be completed through FinalForms prior to Band Camp. Directors have been instructed not to collect any medical paper documentation.  Parents and students are responsible for their paper physicals and uploading them to FinalForms.

Per MSHSAA regulations, physicals are valid for two years unless otherwise specified by the examining physician.  If the doctor determines a shorter validity period is necessary, such as one year, that will be noted on the physical.
